GOALS
To address long term care for Aboriginal elders\seniors, disabled and
chronically ill with special needs who require acute care in a way that
is cultural, appropriate, (w)holistic, in home, equitable, on-going,
community based for all "off-reserve" Aboriginal individuals, families,
communities who require a continuum of physical, mental, emotional
spiritual care and support regardless of cultural beliefs, financial
status, age, gender, sexual orientation, geographic location, residency.
The delivery support strategies encompasses promotion, prevention,
treatment, rehabilitative, respite and palliative care.
OBJECTIVES
| To carry out on-going community needs assessments to "off
reserve" Aboriginal Elders\Seniors, Disabled and Chronically ill
with special needs.
To ensure the community support services are promoted and
made available for:
Traditional Congregate Dining
Friendly Visits
Security Checks
Transportation to Doctors
Aboriginal Support Services

To ensure all urban Aboriginal peoples are given fair and reasonable
access to all long term care services.
To liaise with long term care service providers, doctors,
pharmacists, audiologists, homecare services, etc.
To be involved in community development activities with local
support services ensuring continuity through proper contacts
and\or referrals.
To promote education and employment opportunities to the
disabled and chronically ill with special needs.
To provide referrals and liaise with Aboriginal and
Non-Aboriginal service providers as requested by Aboriginal
client and their families.
To recruit, train, and maintain a bank of approved community
volunteer drivers, escorts, friendly visit etc.
To work in conjunction with the N.B.I.F.C. staff and Advisory
Circle.
To ensure an effective and competent Life Long Care Program.
